The Stanford Cognitive Aid Group released on the 10th June 2014, Version 2.0 of their Emergency Manual.
This group, under the guidance of David Gaba, has pioneered the use of simulation and cognitive aids.
The Stanford Emergency Manual (cognitive aids for per-ioperative critical events) was first made available as a free download in 2012.

Ariadne Labs (a collaboration between Brigham and Women’s Hospital and the Harvard School of Public Health) have also created their own collection of Operating Room Crisis Checklists. These were made available for free as a download in 2013.
